The alternate ending to "I am legend" is: Robert Nivelle discovers that the hemocytes just came for the female that he captured before , so instead of blowing himself and the hemocytes up with a grenade he relieves her from the cure and returns her to the hemocytes.

The Alternate Ending of I Am Legend in fact the alternate theatrical version's ending is actually different from the original. Instead of detonating himself and his blood cells with a grenade, Neville realizes that the blood cells actually came from the females he captured earlier in the movie. It dilutes the remedy and restores it to the blood cells.
